i just wanna sleep.
Venting is a very good thing. Can’t say that it made the troubles go away, but I do feel like I may have a little bit of a game plan (yes, I’m being vague, perhaps more details in the future).
Have to give a presentation on a paper tomorrow. Haven’t found the paper yet. Plus I have clinic. Gosh, I love Tuesdays.
Still loving the ICU (for real, no sarcasm here), but it’s freaking me out. Starting to sink in that I’m the one “in charge.” Even though I have a fellow as backup, he’s not always there, and if something goes wrong, I’m expected to step up and direct and manage until he gets there. Plus I have 2 fourth year students that I’m responsible for… and they’re making decisions that I have to constantly evaluate to make sure they are appropriate. Plus, I have to know about every patient on the team, which even though it’s a small team right now (3 patients. 1 consult. 2 of the patients are palliative. All that equals no fun, no procedures for Julia), these patients have been here a long time and that’s a lot of reviewing that I have to do. I’ve already “messed” up once, where my student wrote an order that I should have questioned more closely and didn’t, leading to extra invansive procedures on an already sick guy.
And now, I really have to go track down an article to talk about.

Julia, can I ask you a doctor question? You’re in neurology, right?
Since, like, December I’ve been having facial pain. I’ve thought it was a problem with my glasses, and that I was extra sensitive, but never knew because I wore contact lenses. Since I still cannot wear contact lenses (something that stumps every doctor I’ve gone to see), I am forced to wear glasses. (I should mention that the last guy I saw things that the contacts not only irritated my eyelids, but also “suffocated” the nerves that end on my eyeball.)
In March, as the pain got worse, I decided there was something else going on. No one could possibly be in this much pain from glasses. I went to my university health services and they suggested I was suffering from allergies, because pollen was particularly high this year. I was given nasal steroids and allegra. I got a little better for some time and then worse again.
When I got home in early May, it was really bad again. I went to my doctor and she treated me for a sinus infection, figuring that must be what I had. She also gave me Singulair. I got better for about a week and then worse again. I went back and she treated me again, figuring it takes awhile to go away. I got a little better and then worse again.
I went to an ENT (otolaryngologist) who didn’t suspect I had a sinus infection, but ordered a CAT scan anyway. I got one and he saw no sinus structural damage and figures it must be muscle.
I wasn’t so bad this past week, but got worse again this weekend. On Sunday I was horrible until it rained, and then I was fine. I still thought it was sinus related maybe.
Starting last night it was really bad again. Today was maybe the worst it has ever been. I have piercing pain all across the left side of my face (the side it hurts 80% of the time), all the way from the left side of my nose, around my eyes, and even on a bottom tooth on the left side of my mouth. When the pain has been more mild, it’s felt like sinus pressure or I had been punched in the cheek. Today it feels like that but is more stabbing. I can feel it running across the side of my face (I believe it’s called the Trigeminal –Fifth– Nerve?).
Does this make ANY sense to you? I’m going to have to call my GP and tell her what’s going on and see what she suggests.

I should also mention I’ve seen an allergist: she found nothing I am allergic to.

One last thing: Since November, I’ve been having a problem with my left eye. It clicks when I blink. It feels like I either have bubbles under it, or the muscle of the eyelid is stretched (I almost went to the hospital in Amsterdam in November because I was in so much pain). I don’t know if there’s a problem with the conjunctiva or something, but no eye doctor has focused on it, and the most recent one I’ve seen says he’s “not ready to talk about it because he doesn’t know it is.” I only mention it because there were a lot of google hits with trigeminal neuralgia and conjunctiva.

I can’t really offer any kind of diagnosis over the internet, without getting to examine you, but I do agree that you should talk to your GP. Since it is just one sided, it could easily be neurological (trigeminal neuralgia is one possibilty, atypical migraine is another that’s coming off the top of my head) and she could help arrange for a consult, or try you on meds and see if you improve And for the ophthalo portion–I’m so bad with eyes, I couldn’t even offer any suggestions!

Yeah, I know you can’t diagnose me over the internet, lol. Thanks for writing back. I just knew you were into neurology and was freaking out. My GP doesn’t think it’s trigeminal neuralgia because it’s chronic and on both sides (80% one side, but 20% the other). She thinks it’s still sinus, even though the CAT scan says otherwise. Her leading theory is I’m having a drainage problem with my tearducts, which would relate it back to my eyes. Sigh.

There’s nothing going on, which is exactly the problem. Am getting tired of it all, the dating games as if I were still 16 when in reality, I’m approaching middle age (okay, I guess I’m not that old!). *sigh*
